PLEASE NOTE: DAVEL PHOTOGRAPHY ALLOWS UP TO ONE DESIGN CHANGE TO YOUR CARD'S FONT, COLOR OR STYLE.
ADDITIONAL CHANGES WILL BE PERMITTED FOR A $15 CHARGE EACH.

BACK OF THE CARD IMAGE NUMBER: Some cards have a single image, some have more. Please note your choice.

MIDDLE IMAGE NUMBER (Folded or Tri-fold Cards only) Some cards have a single image, some have more. Please note your choice.

5) WORDING
You can include your name and grad year on the front, or leave blank (PLEASE CHECK DATES, TIMES AND SPELLING OF NAMES)
